നവംബര് 14. ഒരു പ്രമേഹദിനംകൂടി വരവായി. സാധാരണ ദിനങ്ങള് ആചരിക്കുന്നത് ഒരു വ്യക്തിയെയോ ഒരു സംഭവത്തെയോ ഒരുപ്രസ്ഥാനത്തെയോ ഓര്മിക്കാനും ഓര്മപ്പെടുത്താനുമായാണ്. എന്നാല് പ്രമേഹത്തിന് അതിന്റെ ആവശ്യമില്ല. കാരണം, ഓരോ നിമിഷവും പല രൂപഭാവങ്ങളില് പ്രമേഹം നമ്മെ ഭയപ്പെടുത്താന് തുടങ്ങിയിട്ട് നൂറ്റാണ്ടുകള് പിന്നിട്ടിരിക്കുന്നു.
1922-ലാണ് പ്രമേഹ ചികിത്സയ്ക്ക് ഇന്സുലിന് കണ്ടുപിടിച്ചത്. ഇതോടുകൂടി പ്രമേഹരോഗികള്ക്ക് ജീവിതത്തിലേക്ക് തിരിച്ചുവരാന് സാധിക്കും എന്നു തെളിഞ്ഞു. ആ കണ്ടുപിടിത്തത്തിനു പിന്നിലെ ശാസ്ത്രജ്ഞന്മാരില് ഒരാളായ ഡോ. ഫ്രെഡറിക് ബാന്റിംഗിന്റെ ജന്മദിനമാണ് ലോകമെമ്പാടും നവംബര് 14ന് പ്രമേഹദിനമായി ആചരിക്കുന്നത്.
1991 മുതലാണ് ലോക പ്രമേഹദിനം ആചരിച്ചു തുടങ്ങുന്നത്. ഇന്റര്നാഷണല് ഡയബറ്റിസ് ഫെഡറേഷനും ലോകാരോഗ്യ സംഘടനയും ചേര്ന്നാണ് ഇപ്പോള് പ്രമേഹദിനത്തിനായുള്ള ഒരുക്കങ്ങള് നടത്തിവരുന്നത്. പത്തു വര്ഷങ്ങള്ക്കു മുമ്പാണ് ഐക്യരാഷ്ട്രസഭ ഒരു രോഗത്തിനെക്കുറിച്ച് ചര്ച്ചചെയ്യാന് ഒത്തുകൂടിയത്. അത് എയ്ഡ്സിനെക്കുറിച്ചായിരുന്നു. അതിനു ശേഷം 2011 സപ്തംബറില് പ്രമേഹം ഉള്പ്പെടെയുള്ള രോഗങ്ങള് ചര്ച്ചചെയ്യാനും സുപ്രധാന തീരുമാനങ്ങള് എടുക്കാനും ലോകരാഷ്ട്രങ്ങള് ഒത്തുകൂടി.
അര്ബുദത്തെ പോലെ തന്നെ ഗൗരവസ്വഭാവമുള്ള രോഗമായാണ് ആഗോളതലത്തില് ഇന്നു പ്രമേഹത്തെ കണക്കാക്കുന്നത്. ഓരോ എട്ടു സെക്കന്ഡിലും പ്രമേഹം കാരണം ഒരാള് മരണമടയുന്നു എന്ന കണക്ക് ഞെട്ടിപ്പിക്കുന്നതാണ്.
2009 മുതല് അഞ്ചു വര്ഷക്കാലം പ്രമേഹ പ്രതിരോധത്തിനും പ്രമേഹ ചികിത്സയ്ക്കും 'വിദ്യാഭ്യാസമാണ് പരമപ്രധാനം' എന്ന വിഷയമാണ് എല്ലാ പ്രമേഹ ദിനങ്ങളിലും ഇന്റര്നാഷണല് ഡയബറ്റിസ് ഫെഡറേഷന് പ്രചരിപ്പിക്കാന് ലക്ഷ്യമാക്കിയിരിക്കുന്നത്.
മറ്റു സംസ്ഥാനങ്ങളില് നിന്നു വ്യത്യസ്തമായി, കേരളത്തില് പ്രമേഹരോഗികളുടെ എണ്ണം വളരെ കൂടുതലാണ്. രോഗത്തെക്കുറിച്ചുള്ള അറിവുണ്ടായിട്ടും വിദ്യാസമ്പന്നരായിട്ടും ഈ മഹാമാരിയെ ചികിത്സിച്ചു നിയന്ത്രണവിധേയമാക്കുന്നതില് നമ്മള് ദയനീയമായി പരാജയപ്പെട്ടുകൊണ്ടിരിക്കുകയാണ്.
കേരളത്തില് ചികിത്സ ലഭിച്ചു കൊണ്ടിരിക്കുന്ന 76 ശതമാനം രോഗികളിലും രക്തത്തിലെ പഞ്ചസാര, രക്തസമ്മര്ദം, രക്തത്തിലെ കൊഴുപ്പ് (പ്രമേഹ ചികിത്സയുടെ വിജയം നിര്ണയിക്കുന്ന മൂന്ന് പ്രധാന അളവുകോലുകള് ഇവ മൂന്നുമാണ്) എന്നിവ നിയന്ത്രണവിധേയമല്ല എന്ന് ചില പഠനങ്ങളില് കണ്ടെത്തിയിട്ടുണ്ട്.
കേരളത്തില് ചികിത്സ പരാജയപ്പെടുന്നതിനുള്ള കാരണങ്ങള് താഴെ പറയുന്നവയാണ്:
1. ഇന്സുലിന് ഇഞ്ചക്ഷന് എടുക്കുന്ന വിധം, സ്ഥാനം, സമയം തുടങ്ങിയവയിലെ അപാകതകള്
2. പ്രമേഹചികിത്സയ്ക്ക് ഗ്ലൂക്കോമീറ്റര് അത്യന്താപേക്ഷിതമാണെങ്കിലും കേരളത്തില് ഒരു ശതമാനം പേര് പോലും അത് ഉപയോഗിക്കുന്നില്ല. ഉപയോഗിക്കുന്നവരാകട്ടെ അതു വേണ്ട വിധത്തില് പ്രയോജനപ്പെടുത്തുന്നുമില്ല
3. പ്രതിരോധത്തിനായി കഴിക്കേണ്ട ഔഷധങ്ങള് പലതും അല്പ ദിവസങ്ങള് മാത്രം ഉപയോഗിച്ച് നിര്ത്തുന്നു
4. വ്യായാമം എത്രത്തോളം, എപ്പോള്, എങ്ങനെ ആകാം എന്നത് മനസ്സിലാക്കാതെ അതു നടപ്പിലാക്കുന്നു
5. രക്തത്തിലെ പഞ്ചസാര വളരെ കാലം ഉയര്ന്നുനിന്ന ശേഷം വൈകി ഇന്സുലിന് ഇഞ്ചക്ഷന് തുടങ്ങുന്നു
6. തുടര്ച്ചയായ ഉത്സവങ്ങള്, ആഘോഷങ്ങള്, മറ്റു പരിപാടികള് ഇവ കാരണം ഭക്ഷണനിയന്ത്രണങ്ങള് പാലിക്കാന് കഴിയാതിരിക്കുക... ഇങ്ങനെ നിരവധി കാരണങ്ങളാണ് ചികിത്സ പരാജയത്തിന് വഴിയൊരുക്കുന്നത്.
പ്രമേഹത്തിന്റെ പ്രാധാന്യവും അതുമൂലം ഉണ്ടാകുന്ന മറ്റു രോഗങ്ങള് ചികിത്സിക്കാനായി ഭാവിയില് വേണ്ടിവരുന്ന വലിയ ചികിത്സാചെലവും ഇപ്പോള് മലയാളികള്ക്ക് അറിവുള്ളതാണ്. പ്രമേഹം വരുത്തിത്തീര്ത്ത വൃക്കസ്തംഭനം, ഹൃദയാഘാതം, അന്ധത ഇവയൊക്കെ ചികിത്സിക്കാന് ഇപ്പോള് ആശുപത്രികളില് വന് തിരക്കാണ്. എന്നാല് പലരും അറിയാതെ പോകുന്ന സത്യം, ഈ അനുബന്ധ രോഗങ്ങള് എല്ലാം തന്നെ ഒഴിവാക്കാന് കഴിയുന്നതായിരുന്നു എന്നതാണ്.
രോഗികള് അല്ലെങ്കില് രോഗം വരാന് സാധ്യതയുള്ളവര് ഓര്ക്കേണ്ട കാര്യങ്ങള് ഇത്രമാത്രം: പ്രമേഹം എന്ന രോഗം പാരമ്പര്യസ്വഭാവമുള്ളതാണെങ്കില് പോലും പരിപൂര്ണമായും പ്രതിരോധിക്കാന് സാധിക്കും. അഥവാ പ്രമേഹരോഗം ഉണ്ടെങ്കിലും ശാസ്ത്രീയമായ അറിവ് ലഭിക്കുകയാണെങ്കില് രോഗ ചികിത്സ വിജയിക്കും. അനുബന്ധരോഗങ്ങള് നൂറു ശതമാനവും പ്രതിരോധിക്കാനും കഴിയും. ശാസ്ത്രീയമായി പഠനങ്ങള്ക്കു വിധേയമാക്കാത്ത ഒരു ഔഷധവും ഉപയോഗിക്കാന് പാടില്ല.
പ്രമേഹത്തിന് ചെറുതും വലുതും എന്ന വേര്തിരിവില്ല. ആരംഭത്തിലാണെങ്കില് പോലും ശരീരത്തിലെ എല്ലാ അവയവങ്ങളും സമഗ്രപരിശോധനയ്ക്കു വിധേയമാക്കി വേണം ചികിത്സ തുടങ്ങാനും തുടരാനും. ലോക പ്രമേഹദിനത്തോടനുബന്ധിച്ച് ആഗോളതലത്തില് നിരവധി ആഘോഷങ്ങള് നടക്കുകയാണ്. സൗജന്യ ക്യാമ്പുകള്, വിദ്യാഭ്യാസ പരിപാടികള് എന്നിങ്ങനെ പല പദ്ധതികളും കേരളത്തിലെ പല ആതുരാലയങ്ങളിലും നടന്നുവരുന്നു. പ്രധാന കെട്ടിടങ്ങള്, ചരിത്ര സ്മാരകങ്ങള് എന്നിവ നീലപ്രകാശത്തില് അലങ്കരിച്ച് പ്രമേഹദിന സന്ദേശം കൂടുതല് വ്യാപകമാക്കാനാണ് ഇന്റര്നാഷണല് ഡയബറ്റിസ് ഫെഡറേഷനും ലോകാരോഗ്യ സംഘടനയും ആഹ്വാനം ചെയ്തിരിക്കുന്നത്. 'പ്രമേഹത്തിനെതിരെ പ്രതികരിക്കൂ, ഇപ്പോള്ത്തന്നെ'- ഇതാണ് ഈ വര്ഷത്തെ സന്ദേശം.
പ്രമേഹരോഗം വരുത്തിത്തീര്ത്ത അനുബന്ധരോഗങ്ങള് കേരളത്തിന്റെ സാമൂഹിക, സാമ്പത്തിക, ആരോഗ്യമേഖലയെ ഗുരുതരമായി ബാധിച്ചു കഴിഞ്ഞിട്ടുണ്ട്. നമ്മുടെ വിദ്യാഭ്യാസത്തിന്റെ ശക്തി, അറിവ് ഇവ പ്രമേഹനിയന്ത്രണത്തിനും പ്രതിരോധത്തിനും പരമാവധി പ്രയോജനപ്പെടുത്താന് ഇനിയെങ്കിലും നമുക്ക് സാധിക്കണം.

കരളിനെ സംരക്ഷിക്കാം
അഞ്ഞൂറിലധികം വ്യത്യസ്ത ധര്മങ്ങള് നിര്വഹിക്കുന്ന അതിപ്രധാനമായ ഒരു ആന്തരികാവയവമാണ് കരള്. കേടുപറ്റിയാല് സ്വയം സുഖപ്പെടുത്താനും സ്വന്തം ശക്തിയെ പുനര്ജനിപ്പിക്കാനുമുള്ള ശക്തി കരളിനുണ്ട്. ഇതിനു പുറമേ അസാമാന്യമായ സഹനശേഷിയുമുണ്ട്. രോഗലക്ഷണങ്ങള് ഒന്നും തന്നെ പ്രകടിപ്പിക്കാതെ പ്രവര്ത്തനം തുടരുന്നതിനാല് ഒട്ടുമിക്ക കരള്രോഗങ്ങളും ഏറെ വൈകിയാണ് കണ്ടെത്താറുള്ളത്. 'യകൃത്' എന്ന സംസ്കൃതപദത്താലാണ് ആയുര്വേദത്തില് കരളിനെ സൂചിപ്പിക്കുന്നത്. 'വേണ്ട രീതിയില് സംയമനം ചെയ്യുന്നത്' എന്നാണ് ഇതിനര്ഥം. കരളുമായി ബന്ധപ്പെട്ട് നിരവധി രോഗങ്ങള് വരാറുണ്ട്. വിവിധയിനം മഞ്ഞപ്പിത്തം, കരളില് കൊഴുപ്പടിയുന്ന ഫാറ്റിലിവര്, യകൃദുദരം (സിറോസിസ്), കാന്സര്, പലതരം അണുബാധകള് തുടങ്ങിയ രോഗങ്ങള് കരളിനെ ബാധിക്കാറുണ്ട്.
പ്രതികൂല സാഹചര്യങ്ങള്
മദ്യപാനമാണ് കരളിന്റെ ആരോഗ്യത്തെ തകര്ക്കുന്ന പ്രധാന വില്ലന്. ആഹാരത്തിലൂടെയും മറ്റും ശരീരത്തിലെത്തുന്ന വിഷവസ്തുക്കളെ അരിച്ച് മാറ്റുന്ന അവയവമാണ് കരള്. സ്വാഭാവികമായും മദ്യത്തിലുള്ള വിഷാംശങ്ങളും കരളില് വെച്ച് തന്നെയാണ് വിഘടിക്കപ്പെടുന്നതും. അതുകൊണ്ടു തന്നെ മദ്യപാനത്തിന്റെ കുഴപ്പങ്ങള് ഏറ്റവുമധികം ബാധിക്കുന്നതും കരളിനെയാണ്. മദ്യപാനം കരളിലെയും ആമാശയത്തിലെയും മൃദുകലകളെ ദ്രവിപ്പിച്ച് വ്രണമുണ്ടാക്കുകയും കരളില് നീര്ക്കെട്ടുണ്ടാക്കുകയും ചെയ്യും. കാലക്രമേണ കരളിന്റെ ആകൃതിയും സ്വാഭാവിക ധര്മങ്ങള് തന്നെയും നഷ്ടപ്പെട്ട് അത്യന്തം അപകടകാരിയായ സിറോസിസിലേക്ക് മദ്യപാനിയെ എത്തിക്കുകയും ചെയ്യും.
മദ്യപാനത്തിന് പുറമേ പോഷകക്കുറവ്, പച്ചക്കറികളിലും മറ്റും കലരുന്ന കീടനാശിനികള്, ചിലയിനം മരുന്നുകളുടെ തുടര്ച്ചയായ ഉപയോഗം, കൃത്രിമ ഭക്ഷണം, അമിത ഭക്ഷണം, വ്യായാമത്തിന്റെയും വിശ്രമത്തിന്റെയും കുറവ്, തുടര്ച്ചയായ മാനസിക പിരിമുറുക്കം തുടങ്ങിയവയും കരളിന്റെ ആരോഗ്യത്തെ ബാധിക്കാറുണ്ട്. വര്ധിച്ച തോതിലുള്ള പ്രമേഹവും കൊളസ്ട്രോളും കരളിന് ഹാനികരമാണ്.
രോഗലക്ഷണങ്ങള്
കരളിനുണ്ടാകുന്ന ചില്ലറ പ്രശ്നങ്ങള് അത് സ്വയം പരിഹരിക്കുകയാണ് പതിവ്. അതിനാല് മിക്ക കരള്രോഗങ്ങളും തുടക്കത്തില് കാര്യമായ ലക്ഷണങ്ങള് കാട്ടാറില്ല. അമിത ക്ഷീണം, അരുചി, ഛര്ദി, ശ്വാസത്തിന് ദുര്ഗന്ധം, ശരീരം മെലിച്ചില്, ചൊറിച്ചില്, വയറിനകത്തെ പലതരം അസ്വസ്ഥതകള്, പനി, മഞ്ഞപ്പിത്തം, രോമം കൊഴിയുക, വയറ്റില് വെള്ളം കെട്ടിനില്ക്കുക തുടങ്ങിയവയാണ് കരള്രോഗത്തിന്റെ പ്രധാന ലക്ഷണങ്ങള്.
മെയ്യനങ്ങാതെയുള്ള ജീവിതശൈലിയും നിയന്ത്രണമില്ലാതെ അകത്താക്കുന്ന മദ്യവും അമിതമായ കൊഴുപ്പും കൃത്രിമ നിറങ്ങള് ചേര്ന്ന ഭക്ഷണങ്ങളും എല്ലാം തന്നെ കേരളത്തിലും കരള്രോഗികളുടെ എണ്ണം ഗണ്യമായി കൂട്ടി.
ജീവിതശൈലീ ക്രമീകരണങ്ങള്
നിത്യവുമുള്ള വ്യായാമവും പോഷകങ്ങള് നിറഞ്ഞ നാടന് ഭക്ഷണങ്ങളും കരളിന്റെ ആരോഗ്യത്തിന് അനിവാര്യമാണ്. മദ്യപാനവും പുകവലിയും തീര്ത്തും ഉപേക്ഷിക്കണം. കൊഴുപ്പും നീര്ക്കെട്ടും കുറച്ച് കരളിനെ പ്രവര്ത്തന സജ്ജമാക്കാന് വ്യായാമത്തിനാകും. ഡോക്ടറുടെ നിര്ദേശാനുസരണം മാത്രം വ്യായാമം ശീലിക്കുകയാണ് വേണ്ടത്.
